There are two points of evaluation for the co-op, one unofficial and one official

Midterm Performance Review

An informal review (doesn't go on any record) that happens in the middle of the co-ops term (end of 2nd month). It should be a mock of the final performance evaluation, using the Student Performance Evaluation Form. It is the responsibility of the Day-to-Day Manager of the Co-op to complete this review with the co-op. They can also have the Co-op Supervisor attend if there is anything they want to talk over with them at the review.

End of Term Performance Evaluation

 A formal evaluation of the co-op's quality and quantity of work throughout the term. It will again use the evaluation form, but this time will be entered into the WaterlooWorks website and stay on the co-op's student official record which future employers will see. The evaluation will be emailed to the Supervisor by WaterlooWorks during the final 2 weeks of the co-op's term. It must be completed by the end of the co-op's employment. 

How it is done

  1. First, the Supervisor received the performance eval ready email from WaterlooWorks. They forward it to the Day-to-Day Manager.
  2. The Day-to-Day Manager completes the evaluation form based on their evaluation of the co-op's work, but does not submit it, only saves it as a draft.
  3. The Supervisor and Manager have a consensus meeting and come to a consensus on the evaluation of the co-op.
  4. The Supervisor, Manager, and Co-op have a meeting to go over the evaluation with the co-op. Everyone must be on the same page on what and why is being submitted before it is submitted.
  5. The evaluation is submitted by the Manger, after the Supervisor signs it.
